OS Digital Accessibility Conformance Review Intake Form

The intake form is for submitting content to be reviewed by OS Digital Accessibility Subject Matter Experts (SMEs) for conformance status. Note: Only OS owned or managed content will be accepted.

Content owners and/or authors should review the HHS Accessibility Conformance Checklists before submitting content.

Instructions:

  1. Download and save the form for each new submission. This action ensures the most current form is used.
  2. Complete ALL required fields. Important: Incomplete forms will not be accepted.
  3. Follow all form instructions.
  4. Submit the completed form by email to OS Digital Accessibility Service Desk, opens in a new window with any applicable attachments or questions.
