Whistleblower Tips and Complaints Regarding the Chemical and Surgical Mutilation of Children

On April 14, 2025, the U.S. Department of Health and Human Services (HHS) published guidance to protect whistleblowers who make reports regarding the chemical and surgical mutilation of children. Please use read this guidance prior to tip or report using the form below.

HHS is committed to protecting whistleblowers to the full extent of the law.

You have three options to report a tip or complaint:

(1) You may use the submission form on this page to report a tip or complaint related to the chemical and surgical mutilation of children or whistleblower retaliation, which is limited to information that does not include protected health information (PHI).

(2) For any tip or complaint related to fraud, abuse, or misconduct related to Medicare, Medicaid, or other HHS programs, you may call the HHS Office of Inspector General (HHS-OIG) hotline, available at https://oig.hhs.gov/fraud/report-fraud/. This hotline may be used to submit information that includes PHI and should only be used for complaints related to fraud, abuse, or misconduct in HHS programs.

(3) For any complaint involving conscience protections, you may use the HHS Office for Civil Rights’ complaint portal.

Investigations are most successful when you provide as much information as possible about the allegation and those involved. The more you tell us, the better chance we can determine whether an investigation can be pursued.

Submit a Tip or Complaint

You can use this form to submit a tip or complaint regarding the chemical or surgical mutilation of children to HHS.

Please do not disclose any protected health information (PHI) using this form. If your tip or complaint involves such information and involves fraud, abuse, or misconduct in an HHS program, please use the HHS OIG hotline. See HHS Summary of the HIPAA Privacy Rule and 45 C.F.R. § 160.103 for more information on what constitutes PHI.
