HHS Agency Performance Plan and Report

The Agency Performance Plan (APP) provides information on HHS' planned strategies for advancing progress towards the HHS Strategic Plan Goals and Objectives over the next year.

The Agency Performance Report (APR) provides information on HHS’s progress toward achieving the goals and objectives described in the HHS Strategic Plan and Annual Performance Plan.

Where possible, HHS combines the APP and APR. Those documents are labeled with the Fiscal Year (FY) of the Agency Performance Plan.

Read the FY 2027 Agency Performance Plan and Report (Released April 2026).
