Guidance for: The CY 2009 Formulary Submission Module and Reports Technical Manual has been updated to include instructions on the submission and file formats of the supplemental formulary files: Free First Fill File, Gap Coverage File, Home Infusion Drug File, Excluded Drug File and Over the Counter Drug File.
