Provider Reimbursement Manual - Part 2, Provider Cost Reporting Forms and Instructions, Chapter 39, Form CMS 287-05
This transmittal is a new Chapter 39, Home Office Cost Statement, Form CMS 287-05. The intent
of this major revision is to update the forms and instructions, eliminate obsolete items and correct
inconsistencies. The effective date for the instructional changes will be for cost reporting periods
ending on or after September 30, 2005.
