Pharmacy Accreditation Exemption Statement Fact Sheet
Guidance for criteria that must be fulfilled by pharmacies as of January 1, 2011 in order to be eligible to file an accreditation exemption statement. This statement enable pharmacies to enroll in Medicare to supply durable medical equipment, orthotics, prosthetics and supplies (DMEPOS).
