LTSS Roadmap Planning

Guidance for establishing long-term services and supports or improving a current program using the LTSS roadmap planning model.

Final

Issued by: Centers for Medicare & Medicaid Services (CMS)

Issue Date: August 03, 2016

The LTSS roadmap planning model can guide you through the planning process. The roadmap will help you understand your community's needs and work toward meeting them—whether you have an active program already or are beginning the planning process for the first time.

The roadmap model

The roadmap offers a step-by-step planning process for building an LTSS program for your community.

It will help you plan effectively by considering:

  • LTSS needs
  • Program goals
  • Program design
  • Potential resources
  • Opportunities for collaboration
