Invitation to Provide Feedback on the Contract Year 2015 Formulary Submission
Guidance for inviting all Part D sponsors to provide feedback on the Contract Year (CY) 2015 formulary submission process in the Health Plan Management System (HPMS). Specifically, we are seeking comments and suggestions regarding the following areas: Formulary Crosswalk; Formulary Deadline Date Change; Formulary File Submission; Formulary Reports; Formulary Review Process; Outlier Justification; P &T Attestations; Prior Authorization (PA) & Step Therapy (ST) Attestations; Supplemental Formulary File Submission; Transition Policy Submission; Utilization Management Criteria Submission (PA, ST); Additional Demonstration Drug (ADD) File Submission (Medicare-Medicaid Plans Only).
