To provide a historical overview of the participating hospitals, before the first project report, Premier Healthcare Informatics has used data already available for three of the five conditions from October 2002 - September 2003. This historical data does not affect project incentives.

For this report, historical data for calculating the overall quality score is available for three conditions: Acute myocardial infarction (AMI), Heart failure (HF), and Community-acquired pneumonia (CAP). All indicators are available for pneumonia, except "initial antibiotic consistent with current recommendations" and "influenza screening/vaccination."

Only hospitals that had available data for the historical period are reported, not the entire population of project participants. In some instances, hospitals had a low sample size or did not perform a procedure. Those instances are noted.
