A health center’s scope of project defines the activities supported by the Health Center Program project budget. Specifically, scope of project defines the approved service sites, services, providers, service area(s) and target population(s). This page provides information on how to accurately document scope of project and when to submit a Change in Scope (CIS) request to HRSA.

What is Change in Scope?

CIS requests may include the following: adding a new service or service delivery site, deleting a service or service delivery site, or adding a new target population.   

Some proposed changes to a health center's approved scope of project require prior approval from HRSA in EHBs. Health centers should submit CIS requests online through EHBs at least 60 days before implementing a change.

Preparing and Submitting a Change in Scope Request

Step 1: Review resources, sample forms, and instructions below to help you determine whether and how to submit a request to change your health center’s scope of project.

Step 2: Consult with health center Project Officer on proposed change.

Step 3: For formal CIS requests, review PAL 2014-10: Updated Process for Change in Scope Submission, Review and Approval Timelines (PDF - 127 KB).

Step 4: Utilize resources, sample forms, and instructions below to help you gather the information and documentation necessary to submit the CIS request through EHBs.

Step 5: Complete and submit the official forms online through EHBs.

Step 6: Once a formal Change in Scope request has been submitted, reviewed, and approved, a Notice of Award (NoA) (for grantees) or a HRSA notification (for look-alikes) will be issued.

Step 7: Health centers must then verify in EHBs that the change has been implemented within 120 days of HRSA’s NoA/notification (see Step 6). Once verified, the change will be officially documented in the health center's approved scope of project.

* Health centers are reminded to consider the impact of a Change in Scope request and to ensure the accuracy of their scope of project for the purposes of Medicaid and Medicare FQHC reimbursement, FTCA coverage (eligible to grantees only), and 340B Drug Pricing Program benefits, as appropriate. While identification within a scope of project is required for participation in these programs, it is not a guarantee that these benefits will be realized. Each of these programs has a specific application process and a comprehensive set of requirements, of which scope of project is only one.
