Calendar Year (CY) 2005 Participation Enrollment and Medicare Participating Physicians and Suppliers Directory (MEDPARD) Procedures
Carriers conduct an enrollment period on an annual basis in order to provide eligible physicians, practitioners and suppliers with the opportunity to enroll in, or terminate enrollment from, the participation program. This instruction furnishes carriers with the materials needed for the 2005 participation enrollment effort.
