Question: What form does a Payee Group's Financial Institution provide directly to the Centers for Medicare & Medicaid Services (CMS)?
Answer: Financial Institutions must submit a Bank Verification Letter (BVL) directly to CMS by facsimile at 443/380-5196. This Letter must be on official bank letter head and contain the Issuer's name on the account, banking account type (checking/savings), ACH routing transit number, bank account number and an authorized bank officer's name as well as the bank officer's signature and contact information.
