Question: Can the Centers for Medicare & Medicaid Services (CMS) confirm that Issuers cannot use a P.O. Box address in the Financial Information Form (FIF)?
Answer: Issuers may not enter a P.O. Box address in either the 1099 Address or Billing Address fields on the FIF unless the United States Post Office does not deliver mail to the street address.
