Summary of Benefits and Coverage (SBC) Template (Standard Format) (HHS, Labor, Treasury)
Guidance for the disclosure requirements under section 2715 of the Public Health Service Act to help consumers better understand their health coverage, as well as compare coverage options. This version of the SBC template is to be used to create SBCs beginning on or after April 1, 2017 and before January 1, 2021.
