LTCH CARE Data Set-Based Quality Measures for the LTCH Quality Reporting Program
Guidance for definitions used to describe the methodology employed by the Centers for Medicare and Medicaid Services (CMS) to select assessment records that are used to compute each Quality Measure (QM) from data collected by Long-Term Care Hospitals (LTCHs) and submitted to CMS using the Long-Term Care Hospital Continuity Assessment and Record Evaluation Data Set (LTCH CARE Data Set) under the LTCH Quality Reporting Program (LTCH QRP).
