Internet-Only Manual (IOM) Update(s) to Publication 100-04, Chapter 23, Section(s) 20.3, 20.4, 20.8 and Addendum - Medicare Physician Fee Schedule Database (MPFSDB) File Record Layout and Field Descriptions
This Change Request (CR) is to update the IOM to clarify that all Healthcare Common Procedure Coding System (HCPCS) Level II codes remain effective until otherwise announced, and to revise the MPFSDB File Layout to accommodate two different conversion factors as a result of the implementation of the Quality Payment Program (QPP).
