The National Partnership to Improve Dementia Care in Nursing Homes (the National Partnership) is committed to improving the quality of care for individuals with dementia, living in nursing homes. The National Partnership has a mission to deliver health care that is person-centered, comprehensive and interdisciplinary with a specific focus on protecting residents from being prescribed antipsychotic medications unless there is a valid, clinical indication and a systematic process to evaluate each individual’s need.

A toolkit that offers education and person-centered, practical interventions that nursing home administrators, directors of nursing, and bedside staff can implement to promote high-quality sleep for residents living with dementia.

Assessments & Practice Guidelines

  This website includes information and resources to help clinicians, providers, and consumers better understand how to manage problem behaviors and psychosis in people with dementia using evidence-based approaches.

  This includes approaches and tools for primary care providers.

  Practice guidelines on the use of antipsychotics to treat agitation or psychosis in patients with dementia.

  This implementation guide was developed by CaringKind, The Heart of Alzheimer’s Caregiving with the assistance of Comfort Matters®, as an outcome of the pilot project, Palliative Care for Advanced Dementia, Training and Implementation, the study of effective ways to implement the Comfort Matters® approach in three New York City nursing homes.

  A systematic review report, which covers following off-label uses of atypical antipsychotic medications for conditions, including dementia (Agency for Healthcare Research and Quality).

  A systematic review article which assesses the effectiveness of interventions used to reduce inappropriate prescribing of antipsychotic medications to individuals with dementia in care homes (Journal of the American Medical Directors Association).

  A report published by the World Health Organization and Alzheimer’s Disease International to raise awareness of dementia as a public health priority, in addition to articulate a public health approach and to advocate for action at international and national levels.

  A research article quantifying the influence of a large-scale communication education training program (OASIS) on nursing home antipsychotic use (JAMA International Medicine).

  The American Association for Geriatric Psychiatry is a membership association of geriatric psychiatrists and other health care professionals in the United States, Canada, and abroad, dedicated to the mental well-being of older adults. 

  A free collection of expert-contributed geriatric educational materials for educators and learners using an array of teaching methods, which are available digitally.

  An international, non-profit 501(c)3 organization dedicated to creating quality of life for elders and their care partners, wherever they may live.

  The NADRC provides technical assistance to grantees, as well as information and resources available to individuals and organizations outside the Alzheimer’s grantee community on Alzheimer’s disease or dementia.

  QAPI tools and resources for the development and sustainably of an effective QAPI program for improving the quality of life, and quality of care and services delivered in nursing homes.  

  The GeriatricPain website provides nurses and other staff working as well as the interdisciplinary team in long-term care environments/geriatric care settings and informal caregivers with access to free evidence-based pain assessment tools, pain management strategies, and resources to help identify and manage pain in older adults.

  The Office on Aging is the focal point for American Psychological Association activities pertaining to aging. Its overarching mission is to promote the application of psychological knowledge to issues affecting the health and well-being of older adults.

  ASCP is a non-profit, membership association devoted to optimal medication management and improved health outcomes for all older adults. ASCP represents pharmacy professionals and students serving the unique medication needs of older adults in varies settings.

  A national non-profit organization who is known for pioneering culture change and person-directed care in long-term care.

  A website that provides consumers and caregivers with up-to-date information on health and aging.

Articles

  Based on two publications by the American Geriatrics Society, this guide was developed to assist healthcare providers in managing psychotic symptoms and disorders in older adults.

  An explicit list of PIMs that are typically best avoided by older adults in most circumstances or under specific situations, such as in certain diseases or conditions.

  Located in rural New Hampshire, Genesis Lebanon Center, both a rehabilitative and long-term care facility outlines the incorporation of new initiatives with a “Just Do It” attitude approach towards the reduction of unnecessary antipsychotic usage.
