Updates

March 2026

On October 1, 2025, the Centers for Medicare & Medicaid Services (CMS) implemented the Hospice Outcomes and Patient Evaluation (HOPE) tool, which replaced the HIS. CMS also launched the Internet Quality Improvement and Evaluation System (iQIES) functionality to submit HOPE records and access reports. The migration for submission of HOPE data into iQIES successfully occurred on October 1, 2025. 

 

Hospices were allowed to submit corrected or late HIS records with target dates of September 30, 2025, or earlier in QIES through February 15, 2026.
The QIES Assessment Submission and Processing, or ASAP system was decommissioned and the data entry tool known as HART, has also been retired.

  • For all patients admitted on or after October 1, 2025, only HOPE records are accepted by CMS. 

  • For patients discharged on or after October 1, 2025, providers will complete and submit a HOPE Discharge.
