This page contains links to Supplemental QRUR templates, methodologies, and supporting information for the 2012 Supplemental Quality and Resource Use Reports (QRURs) that CMS issued in June 2014.  CMS made the 2012 Supplemental QRURs available to group practices with 100 or more eligible professionals (EPs).  The 2012 Supplemental QRURs reflected performance on episodes during a calendar year 2012 performance period.  The 2012 Supplemental QRURs continued to build on research to construct episodes of care and attribute them to medical group practices.  The 2012 Supplemental QRURs were for informational purposes only and complemented the per capita cost and quality information provided in the QRURs. 

CMS will continue to create new reports based on the improved version of the grouping algorithm that is under development.  On the archive pages, we will retain past versions of report templates and methodologies.
