If an entity is represented by a Third Party and will use their bank account information, who should fill out the Payee and Payer modification forms?
Answer:
If a Third Party is making the payments on behalf of the drug manufacturer/sponsor, the Payee and Payer modification forms can be filled out by either the Third Party or the drug manufacturer/health plan sponsor and should include the payment information of the Third-Party submitter. The authorized signer designated on the form must be the drug manufacturer/health plan sponsor. Reminder, the drug manufacturer/health plan sponsor is still ultimately responsible for payment
