PCSFN Community Leadership Award Nomination Form

Overview

The President’s Council on Sports, Fitness & Nutrition (PCSFN) Community Leadership Award is given annually to no more than 50 individuals or organizations that provide or enhance opportunities to engage in sports, physical activity, fitness, or nutrition-related programs within a community. The word "community" may refer to a State, organization, sports league or team, business, city, or community-based group.

Deadline and Process

Nominations for 2019 awards will be accepted between January 2 and April 15. Nominations received after April 15 will not be considered. Nominations with incomplete information will not be accepted. Eligible nominations will be reviewed and winners determined by the President's Council members.

Criteria Categories

When submitting a nomination for a Community Leadership Award, please provide a copy of a resume, if the nomination is for an individual, or an “About Us” section, if the nomination is for an organization. In your essay, please provide information that answers the questions/categories provided below, making sure to identify if the nomination is for an individual or an organization, while describing why this individual or organization is deserving of the award.

The categories that the PCSFN Council members will consider are as follows:

  1. Innovation
    1. What need is the organization filling through their program?
    2. What is the gap in services that the organization is attempting to fill?
    3. How is this unique from other programs?
  2. Content
    1. What population does the organization serve?
    2. What area of the Council’s mission is the organization working in, e.g. fitness, sports, physical activity or nutrition?
  3. Reach
    1. What is the reach of the organization? How many people have they served?
    2. Is there something unique about the program that improves its reach to more than one community?
  4. Success
    1. How many years has an organization been working to improve the health and well-being of the community where it operates?
    2. What is the quantifiable evidence that there have been successes in the approach to improving health and well-being?
    3. How has the program been evaluated? Is there data to back up their claims of success?
  5. Miscellaneous
    1. What other awards has the program received in the past?
    2. Did the nominator provide a resume or other document that helps support the nomination?

Include information on the size of the community touched by the individual(s) and the frequency with which the individual(s) interact(s) with this population. The goal of this essay is to provide the full story as to why this individual or organization is deserving of this award. Please assume this is a stand-alone document and include all pertinent information identified in the criteria categories above.*
