PCSFN Event Request Form

(Please submit request at least 8 weeks prior to your event.)

If you are interested in inviting a Council member, our Executive Director, or one of our staff to attend, exhibit, or participate in your event in a role other than as a speaker, please fill out the Event Request Form. Requests for Council participation should be submitted at least eight (8) weeks prior to your event to allow ample time for processing and scheduling of your request.

Please provide as much of the requested information as possible. After we receive the completed form, we will determine if a representative from the Council is available for the event. If we are able to attend, we will contact you to discuss further details. Due to the high volume of requests, please wait to hear from us.
