The Way Forward: Update on the American Health Information Community (AHIC) Successor

The Surgeon General's My Family Health Portrait (MFHP) is a second-generation, web-based resource for family health history. Developed using open source software, it is standards-based, consumer-friendly, and it can be downloaded and customized in existing health IT systems. Speakers will discuss family history in health care, and describe the MFHP and ongoing demonstration projects.

This session demonstrates the second wounded warrior scenario from the Federal Consortium, a multiple-agency participant of the NHIN Cooperative. Federal and private organizations will showcase three live system demonstrations.
Using fictional, but true-to-life, medical encounters and real-time NHIN information exchanges, this session will illustrate how the NHIN supports continuity of care and determination of disability benefits for the wounded warrior patient population.

This session will demonstrate the exchange of information between systems that collect electronic health information and the systems that support healthcare quality measurement, feedback, and reporting. The AHIC Quality Use Case focuses upon three areas: (1) the impact that collection of electronic health information has upon driving quality of care through better, more comprehensive clinical information at the point of care; (2) measuring and reporting quality with a minimum of burden assessed on the provider; and (3) aggregating health information for the purpose of public reporting of quality.

This session will demonstrate the value of building a consolidated medication history from a patient's self-reported information and data from the patient's pharmacy. In addition, we will demonstrate the use of e-prescribing in an ambulatory setting and upon inpatient discharge, with these medications accessible by physicians caring for the patient post-discharge. Imbedded clinical decision support will highlight the identification of potential adverse drug events.
