Description

Grants will offer States a menu of opportunities to develop specific tools to support the continuing efforts in rebalancing long-term care systems.  These tools may concentrate efforts to improve access to services, improve quality, address barriers to self-directed services and specifically address particular barriers in each State's efforts to create systems change.
