Description

This FOA will solicit grant applications (R03 or R21) for pilot studies that contribute to improvement in the ability of clinicians and patients to make informed decisions about the appropriate timing and provision of therapeutic and preventive services in individuals with two or more chronic conditions. Given the aging of the U.S. population, the number of Americans living with multiple chronic conditions is expected to grow precipitously.  However, nearly all treatment guidelines currently in use are specific to a single condition and fail to take into consideration the possibility of one or more co-morbidities.  Most recommendations for preventive services target the asymptomatic patient and fail to provide guidance on the preferred timing and relative appropriateness of these services for patients whose care is complicated as a result of multiple chronic conditions. AHRQ seeks pilot studies that explore both the relative benefits and harms of providing evidence-based services (testing and treatment) for patients with multiple co-morbidities, and the appropriate timing and frequency of recommended preventive services for these patients.

Proposals may be for observational studies or pilot studies of brief practice interventions.  Of particular interest are modeling projects that use large population-based data sets to estimate how the application and timing of specific, recommended therapeutic and preventive services will affect overall outcomes for the complex patient, especially as the patient ages.  All studies must consider how to optimize the simultaneous management of at least two chronic conditions and the provision of at least one preventive intervention.  Studies assessing mental health co-morbidities are encouraged. Investigators will be required to describe

  1. how the results of the proposed work is likely to affect the healthcare decisions made by clinicians and their patients with multiple chronic conditions, and
  2. if/how current quality of care measures may need to be adjusted as a result of their findings.
