Description

This funding opportunity will support implementation of an electronic health record (EHR).This is in support of the President's Health Center Initiative and goal of universal adoption of electronic health records by 2014.OHIT sees EHRs and other forms of HIT as tools to improve quality of health care and health outcomes, not an end in and of itself.It is expected that the implementation of the EHR will take place in the context of the HRSA Quality Initiative.This will also include networks interested in spreading their capacity to other centers.At the end of the project period, grantees must demonstrate that they have implemented EHRs in the sites that initially proposed. Grantees must also demonstrate how this implementation has or will lead to improvements in health outcomes.All grantees must demonstrate steps taken to achieve sustainability of initiative.

Eligible Entity(ies)

Public and non-profit organizations, including faith based and community based organizations, from one of the following categories: -a network controlled by and acting on behalf of the health center(s), as defined and funded under section 330(e)(1)(C) of the PHS Act. or,-a health center, as defined and funded under section 330 of the Public Health Service (PHS) Act (42 U.S.C. 254b), as amended,applying on behalf of a managed care network or plan, that has received federal grants under subsection330(e)(1)(A) for at least the two consecutive preceding years; or, -a health center as defined and funded under section 330 of the Public Health Service (PHS) Act (42 U.S.C. 254b), as amended, applying on behalf of a practice management network, that has received federal grants under subsection 330(e)(1)(A) for at least the two consecutive preceding years.
