Description | The purpose of this activity is to support the establishment of new service delivery sites for medically underserved populations to receive comprehensive primary and preventive health care services. This activity is a key factor in the President's Health Centers Initiative, a multi-year plan to strengthen the health care safety net by establishing or expanding health centers in 1,200 of the Nation's neediest communities through Health Centers funded under Section 330 of the Public Health Service Act. Through the Health Center Program, funded organizations will offer access to comprehensive primary and preventive health care and social services (including mental health, substance abuse, and oral health care services) to populations currently with limited or no access to such services. Federally funded health centers may offer services to the general community; migrant and seasonal farm workers and their families; homeless people, including children and families; and/or public housing residents. All services must be provided to all persons without regard to an individual's ability to pay. Each application for support to establish a new access point must identify a population in need of primary health care services and propose a specific plan to increase access to care and reduce disparities identified in the population or community to be served. |
Eligible Entity(ies) | Public and non-profit private entities, including tribal, faith-based and community-based organizations are eligible to apply for this funding. Applicants are expected to demonstrate compliance with the requirements for the specific type of health center for which funding is requested. Requirements are found in section 330(e) of the PHS Act for Community Health Centers; section 330(g) for Migrant Health Centers; section 330(h) for Health Care for the Homeless Centers; and section 330(i) for Public Housing Primary Care Centers.
