Prepare a “Keep It With You” form that includes information about your current health status, medications, allergies, and test results that can be used in an emergency. Your health care worker may be able to help you fill out important information in advance of an emergency. Keep the completed paper in your purse or wallet. You may want to consider using a plastic baggy to keep the form safe from water.
