There are two ways of getting a free print version of "My Family Health Portrait": 1) Download the PDF A ready-to-print version of the "My Family Health Portrait" is available for download as a PDF. To order copies through the web, go to www.ask.hrsa.gov, click on "Search the Information Center." When you arrive at the "Welcome to the on-line publications page…" go to "search by the title" and type in "My Family Health Portrait" and hit go. On the "Publications page," click on the title of the publication you wish to order. When you arrive on the "Order publication page", choose your order option. Next, click on the shopping cart icon at the top of the page and proceed to check out. |
